Below you will find what I need from you to get started along with some FYI’s that you may find helpful through our journey together.

Please review and sign attached Service Agreement Document in your email.

Please pay invoice as soon as possible and at least 72 hours prior to scheduled intake.

​

Please be sure to review all aspects of the Service Agreement document especially around payment and cancellations.
